It is very hard to tell by the picture but I would recommend taking mrs.turtle to the vet for a good exam. Most health issues with turtles are based on poor husbandry. I would make sure you are using UVB and UVA light- Most times you have to change the light bulb quite often. Also calcium is very important for the shell development. It is possible that Mrs. turtle needs more calcium. Turtles can get shell rot which is where fungus develops on the shell. You can try to dry dock the turtle by taking it out of water 3 times a day for 20 minutes to try and help the shell dry out
