Had our dog neutered five days ago. Last night he was able to get to his incision and it started bleeding a couple drops of blood, but stopped. It looks weird, almost like a small hole in the middle, and I’m wondering if he opened stitches. Can someone tell me if this is open, and needs attention?

Based on the picture, I think Doc's incision looks okay. The edges aren't quite as well apposed as I'd prefer, but it doesn't look open enough to warrant further treatment at this time. I also don't see any obvious signs of infection, such as significant inflammation, redness, or discharge. Be sure to keep an e-collar (cone) on Doc at all times for the next week and continue to keep a close eye on the incision. If you notice redness, discharge (aside from the few drops of blood you saw yesterday), or swelling, Doc will need to follow up with his vet. I hope the rest of his recovery goes smoothly!
